What is Natural and Mild IVF?

For many people, the idea of conventional IVF feels overwhelming: the injections, the side effects, the physical and emotional toll of high-dose stimulation. Natural Cycle IVF and Mild IVF exist because that experience isn’t right for everyone.

What’s the difference between Natural IVF, Mild IVF, and Standard IVF?

If you’re trying to understand your options, it helps to see all three side by side, because the differences matter.

Standard IVF uses high doses of hormonal medication to stimulate your ovaries to produce as many eggs as possible, typically over 10–14 days. The logic is straightforward: more eggs means more chances of creating embryos, and more embryos mean more chances of a live birth.

For many people, that’s absolutely the right approach, especially as you get older. But it also means more medication, more side effects, a higher risk of Ovarian Hyperstimulation Syndrome (OHSS), and, for some, a harder physical and emotional experience.

Natural Cycle IVF takes the opposite approach entirely: no stimulation drugs at all. It follows your natural menstrual cycle and collects the single egg your body has already chosen. It’s as close to natural conception as IVF can get.

Mild IVF sits between the two. It uses a low dose of stimulation medication, significantly less than conventional IVF, and for a shorter period, with the aim of producing a small number of high-quality eggs (typically 2–10) rather than the highest possible number.

In both Natural and Mild IVF, the philosophy is quality over quantity, and a gentler experience for your body and mind. They’re not the right choice for everyone, but for the right person, they can be the more effective and far less gruelling path.

What is Natural Cycle IVF, and who is it for?

In standard IVF, high doses of hormones are used to stimulate your ovaries to produce multiple eggs at once. Natural Cycle IVF takes the opposite approach: no drugs, no stimulation, and complete respect for what your body is already doing.

It works with the one egg your body has naturally selected that month. That egg is collected, fertilised in the laboratory, and, if all goes well, transferred back.

Natural Cycle IVF might be right for you if:

  • You have a very low ovarian reserve (low AMH), and conventional IVF is unlikely to produce many eggs anyway
  • You’ve had a poor response to stimulation in previous IVF cycles
  • You want to avoid hormonal medication entirely, whether for personal, medical, or emotional reasons
  • You have severe endometriosis, where stimulation can sometimes worsen symptoms
  • You are at increased risk of hormone-sensitive cancers due to your family history

The pros of Natural Cycle IVF:

  • No drugs. This is the most significant distinction. There are no injections, no stimulation-related side effects, and no risk of Ovarian Hyperstimulation Syndrome (OHSS), a potentially serious complication that affects up to 30% of people in conventional IVF to some degree.
  • Less physical and emotional burden. The psychological impact of IVF is real. Research consistently shows that high stimulation has a measurable effect on well-being. Natural IVF, by contrast, tends to be a significantly less stressful experience.
  • You can try again sooner. Because your ovaries aren’t being pushed hard, recovery is quicker and repeat cycles can begin the following month.

The cons of Natural Cycle IVF:

It’s important to understand that Natural Cycle IVF has a significantly lower per-cycle success rate than stimulated IVF.

  • In only 50–70% of cycles will an egg be collected and an embryo available to transfer
  • Around 30% of cycles are cancelled due to premature ovulation, the body releasing the egg before it can be collected
  • Because only one egg is involved, there are usually no embryos left to freeze for future attempts

This doesn’t make it the wrong choice; it means the decision needs to be made thoughtfully, considering all factors, including your future family goals.

What is Mild IVF, and who is it for?

Mild IVF sits between Natural IVF and conventional IVF. It uses a lower dose of stimulation medication for a shorter period, usually 5 to 9 days, with the full treatment cycle taking around two weeks, with the goal of producing a small number of high-quality eggs rather than the largest possible number.

It’s the middle path: more eggs than Natural IVF, but a gentler process than standard IVF.

Mild IVF might be right for you if:

  • You have a reduced ovarian reserve (low AMH), and high stimulation is unlikely to help
  • You have PCOS, where heavy stimulation carries a higher OHSS risk
  • Male factor infertility is the primary reason for IVF (meaning egg quality and quantity are less of a concern)
  • You want a less physically and emotionally demanding treatment, without giving up the benefit of having more than one egg

The pros of Mild IVF:

  • Fewer side effects than conventional IVF, while still producing more eggs than Natural IVF for better chances per cycle.
  • Reduced risk of OHSS, which is particularly important for people with PCOS, where the ovaries are more sensitive to stimulation.
  • A more manageable emotional experience. A shorter, lower-intensity treatment cycle means less disruption to your daily life and, for many, a less anxious one.

The honest cons of Mild IVF:

  • Lower success rates per cycle than conventional IVF
  • Fewer eggs collected, which means fewer embryos for transfer and less to freeze for future babies
  • If multiple cycles are needed, cumulative costs can exceed those of standard IVF, so this needs to be factored into your planning

Can I do multiple Natural IVF cycles in a row?

Yes, because your ovaries aren’t being heavily stimulated, recovery is faster and cycles can often resume the following month. For some people, particularly those with very low reserves, this cumulative approach can be very effective.

What are Mild IVF success rates compared to conventional IVF?

Per cycle, yes, Mild IVF typically produces fewer embryos than conventional IVF. But for people with low ovarian reserve, or those for whom high stimulation causes poor quality eggs, Mild IVF can actually produce better results. The right answer depends entirely on your individual situation.

Will Natural IVF work if I’ve failed conventional IVF before?
